Hospital Social Worker – Advertisement

Job Introduction

Job Title: Hospital Social Worker

Starting Salary: £38,220 – £41,771 per annum, plus £1,911 – £2,088 market enhancement pay, plus 5% operational allowance per year, plus £1,000 welcome payment (relocation and lease car terms apply – pro rata for part-time).

Hours: 37 hours per week Location: Hertfordshire Contract Type: Permanent Directorate: Adult Care Services

Teams Available

East & North Herts Transfer of Care Team

  • Location: Lister Hospital (Stevenage) & Princess Alexandra Hospital (Harlow)
  • Hours: 37 p/w | Permanent
  • Role: Work in acute and community hospitals. Assessments focus on discharge to assess beds and transfer of care.

We are unable to accept AYSE Social Workers at this time.

Integrated Discharge Service

  • Location: Barnet & Chase Farm Hospital
  • Hours: 37 p/w | Permanent

Focusses on integrated discharge planning, working alongside Occupational Therapists, Community Care Officers, healthcare teams, and care providers. You’ll conduct innovative assessments, tailor care plans to individual needs, and support personalisation (enhancing independence, choice, and control).

Gains experience in: adult social care, safeguarding practice, and multi-disciplinary working.

Out-of-County Hospital Integrated Discharge Team

  • Location: Herts Valley (Luton Hospital base, with travel to other hospitals)
  • Hours: 37 p/w | Permanent (1 weekend every 8)
  • Requirement: 7-day service (standard hours: 9:00 a.m. – 5:00 p.m.).

Role: Prompt, safe hospital discharges using a person-centred approach. Discharge to Assess model dominates. Must be willing to travel outside Hertfordshire for residents.


About The Role

Ensuring safe and timely hospital discharges by:

  • Assessing patients and carers’ needs.
  • Arranging appropriate care packages.
  • Liaising with healthcare professionals and support agencies.
  • Supporting person-centred discharge planning.

About You (Essential Criteria)

  • Social work degree.
  • Registration with Social Work England.
  • Full UK driving licence: required (either personal or company lease car access).

Relocation terms apply.
