We are a not-for-profit community interest company. The Confederation works to develop and support individual GP practices, PCNs and Neighbourhoods and their changing needs. We deliver excellent clinical services ourselves both at scale and complementary to General Practice. Job Summary

We have a new and exciting opportunity to appoint Housebound Healthcare Assistants to join our dynamic and friendly operations team. Post holders will be confident in dealing with patients, able to work independently whilst understanding the importance of proactively escalating concerns.

You will be providing the delivery of services including Phlebotomy, 24hr BP monitoring, ECG’s, Warfarin Monitoring, Atrial Fibrillation Monitoring to some of the most vulnerable patients in Hillingdon in their homes. Ideally you will already be skilled in providing phlebotomy services and should be willing to be trained to support the delivery of other aspects of patient care.

You will be supported in your role through friendly management and administration team as well as clinical management for all patient concerns. Working in Hillingdon also means you have access to our outstanding training hub providing support and development to clinicians working in the area.

This will require you to loan work and travel between clinical sites and patients homes within, and up to 1 miles outside of the borough of Hillingdon.

Primary Responsibilities

  • In accordance with the services needs the post-holder will make him/her-self available to undertake a variety of duties including Phlebotomy, ECG, AF monitoring, Post-operative dressings, Pill Checks, Immunisations, Phlebotomy, Spirometry, BP checks, NHS Health Checks and diabetic foot checks according to the agreed and provided processes and protocols. Training will be provided in accordance with business requirements.
  • Assessing the health needs of patients with chronic health needs and using your own judgment to assess whether further input is needed.
  • Ensuring that potential safeguarding concerns are reported immediately
  • Screening patients for disease risk factors and early signs of illness.
  • Contributing to QOF requirements and recording data appropriately.
  • Contributing to PCN requirements and recording data appropriately.
  • In consultation with patients and in line with current practice disease management protocols, developing care plans for health related illnesses.
  • Screening patients for health education.
  • Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Collecting data for audit purposes and contributing to the practice audit programme as required.
  • Take responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date.
  • In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a healthcare assistant working within primary care.

Collaborative Working Relationships

  • Foster and maintain strong links with all services across the PCN and neighbouring networks.
  • Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.

Education, Training and Development

  • Maintain training and updates on the skills expected such as IM injection training, Spiro Updates
  • Complete all mandatory training within expected timelines
  • Attend training as expected to support the delivery of the service and maintain standards

Person Specification

Essential Criteria

  • Eligible to work in UK
  • Able to carry out duties with or without premises adaptation
  • Completed Care Certificate Training or able to demonstrate working towards completion
  • Full Driving licence and appropriate insurance in place and full access to own vehicle
  • Experience of working within the NHS, with general public & team working
  • Well developed IT skills and an enthusiasm for new applications and software
