Role Overview
Job Title: Housing Support Practitioner (Leadership Role) Salary: £27,000 per annum (pro-rata for part-time) Contract: Permanent, Part-time – 12 hours per week (including 1 shift) Location: Caldicot, Monmouthshire

Monmouthshire Domiciliary Care (commissioned by Monmouthshire Council) provides person-centred support to a young person with a disability, helping them build independence, confidence, and a safe home environment.
The Role
Key Responsibilities
As a Housing Support Practitioner, you’ll:
- Provide professional oversight to a small team of Housing Support Workers
- Lead rota management, ensuring consistent staffing and adherence to cover requirements
- Review support plans, risk assessments, and daily notes to maintain quality and accuracy
- Offer reflective supervision, coaching, and practice guidance to staff
- Ensure safeguarding procedures are followed and concerns escalate appropriately
- Collaborate with families, social workers, therapists, and other professionals
- Monitor the young person’s progress and adapt support strategies as needed
- Uphold organisational policies and regulatory compliance

What You’ll Need
Essential Requirements
- A Level 3 qualification in Health & Social Care (or equivalent)
- Proven experience in safeguarding, risk management, and person-centred practice
- Experience supporting children/young adults with disabilities
- Previous work in domiciliary care, supported living, or similar settings
- Experience in leading, mentoring, or supervising staff
- Exceptional interpersonal, organisational, and recording skills
- Strong analytical abilities to make evidence-based decisions
