Clinical Team Lead for Community Rehabilitation

Key Responsibilities
Collaboration & Service Delivery

Work alongside experienced Leads, Physiotherapists, Occupational Therapists, Rehabilitation Assistants, Nurses, Assistant Practitioners, Social Workers, Dietitians, and Speech and Language Therapists to deliver interventions in various settings—supporting and developing Neighbourhood Teams.

Partner with local Commissioners, GPs, and Social Care colleagues to guarantee seamless patient flow between primary, community intermediate, and secondary care.

- Clinically lead a multi-disciplinary team (MDT) within community services, focusing on rehabilitation.
Leadership & Stakeholder Engagement

Develop and maintain positive relationships with:
- Patients and the public
- Staff at all levels, including GPs, practice staff, and third-sector colleagues
- Commissioners and Social Care staff

Provide clinical leadership for the Community Rehabilitation Team, including wider therapy team staff.
Operational & Strategic Accountabilities
Management
-
Effectively operationalise the team, while using resources to meet patient needs.

Ensure alignment with rehabilitation NICE guidelines.

Ensure timely patient flow, minimizing delays.
Performance & Compliance
- Use data and soft intelligence to optimise performance, surpassing Key Performance Indicators (KPIs).
