An exciting opportunity has arisen for a 12-month fixed-term Patient Navigator

To support the development and coordination of a regionally integrated Interstitial Lung Disease (ILD) service. This role will contribute to improving patient experience, clinical outcomes, operational efficiency, and data-driven service delivery.

Responsibilities

  • Working within a specialist multidisciplinary team, the post holder will play a key role in coordinating and managing the ILD patient pathway.
  • This will include tracking patient progress across all stages of care, including the diagnostic phase, proactively pursuing results, and ensuring timely and appropriate follow-up to minimise delays and support high-quality, patient-centred care.
  • Interact with MDT Clinical Teams regarding urgent admissions, outpatient appointments and concerns expressed by patients/relatives and General Practitioners.
  • Communicate with General Practitioners as to the appropriate course of action, i.e. admission or available clinic appointments.
  • Deal with enquiries and problems encountered in the tracking of the patient pathway, communicating with all agencies, which includes taking the appropriate action escalating to the appropriate person as and when required.
  • Ensure timely return of investigations/reports requested by medical staff. Liaising with all staff concerned e.g. medical, nursing staff, Medical Records Department, X-ray Department, Patient Contact Centre, Pathology Department and Management etc.
  • Chase out-standing results and if necessary, liaise with outpatient booking teams to amend patient appointments, making sure follow-up appointments following investigations are arranged where necessary.

Locations

We operate from three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

Furness General Hospital (FGH) in Barrow

  • A range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

  • A range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

Westmorland General Hospital (WGH) in Kendal

  • A range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

Community Facilities

  • All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery.
  • A range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
