Independence and Wellbeing Practitioner (Trustee Assessor)
Suffolk County Council | Hartismere Hospital, Eye, Suffolk, IP23 7DD Salary: £28,598 per annum (pro rata for part-time) Hours: 37 hours per week (flexible working options available) Contract: Secondment or Fixed Term (up to 12 months to cover a member of staff’s secondment)

Your work will focus on:
- Enabling customers to achieve personalised, high-quality outcomes that support independence and wellbeing.
- Assessing functional ability and delivering interventions (e.g., daily living equipment, adaptations, assistive technology, community resources).
- Fostering reablement and digital care solutions to maximise independence.
- Integrating health and social care across multidisciplinary teams for seamless, accessible support.

Your Responsibilities
Core Functions
- Conduct trusted assessments to evaluate customers’ functional needs.
- Provide advice, support, and access to assistive solutions (including adaptations, technology, and community networks).
- Promote digital care solutions and reablement programmes tailored to individual needs.
- Work collaboratively across health, social care, and voluntary sectors to ensure system-wide integration.
Key Priorities
- Advocate for customer rights while explaining eligibility policies and pathways with clarity and empathy.
- Deliver positive, outcome-focused outcomes, aligning with Suffolk County Council’s vision, values, and priorities.
- Foster a culture of continuous improvement, value for money, and long-term independence.
- Lead by example—aligning actions with organisational values in every interaction.

Requirements
Essential
- A genuine passion for empowering individuals to live independently.
- Problem-solving approach and ability to work creatively in interactions (assessments, progress reviews, goal-setting).
- Compassionate listening skills and deep empathy—essential when guiding decision-making.
- Strong understanding of relevant legislation, rights, and pathways (even if not practiced professionally).
- Flexibility and adaptability—navigating diverse needs, from individuals to family systems.
- Teamwork—collaborating with social workers, healthcare partners, carers, and community organisations.
- Resilience and patience—handling complex or high-pressure situations with positivity (e.g., supporting patients through transitions, challenges, or cuts).
Desirable
- Prior experience in adult social care, reablement, disability services, or community work.
- Knowledge of assistive technology, adaptations, or community-based solutions.
- Firsthand experience in integrating multiple systems (e.g., linking care stakeholders).
