Motivated Nurse Needed – Skylark Rehabilitation Unit, Yate

We’re looking for a compassionate Nurse to join our dedicated team at Skylark Rehabilitation Unit, where you can deliver high-quality care to elderly patients and help them regain independence in a supportive environment.

About the Role

Skylark Rehabilitation Unit is a 30-bed community rehabilitation unit in Yate, South Gloucestershire, specialising in the Discharge to Assess Pathway 2. We provide person-centred nursing care and rehabilitation for patients recovering from illness, injury, surgery, or long-term conditions such as frailty, orthopaedic issues, post-operative care, and neurological conditions.

Key Responsibilities

As the Unit Nurse, you will:

  • Assess, plan, implement, and evaluate care programmes for patients with acute and chronic needs, particularly those from the frail elderly cohort.
  • Ensure safe and effective nursing care, maintaining daily routines, including medication rounds, observations, and health promotions like venepuncture.
  • Work on complex care planning, assisting patients in meeting their daily care needs and ensuring safe discharges.
  • Provide 24/7 coverage (primarily day shifts (07:00–19:30) and night shifts (19:00–07:30)), with weekends and bank holidays included as part of the roster.
