The Role
We’re seeking a strategic, collaborative professional to accelerate the integration of Change Grow Live (CGL) services within Camden’s Integrated Neighbourhood Teams (INTs). In this crucial position, you’ll:
- Scope opportunities for CGL’s drug and alcohol services to be fully embedded within multidisciplinary, neighbourhood-based teams.
- Partner with health, social care, primary care, and voluntary organisations to influence systemic integration.
- Drive accessibility, quality, and neighbourhood-level support for individuals facing multiple disadvantages.

Location
Camden, London
Working Terms
- Full-time contract: 37.5 hours per week
- Fixed-term for 12 months, commencing from the start date.
Note: Part-time equivalents will follow a pro-rata salary structure.
Responsibilities
Core Duties
- Identify and champion opportunities to embed CGL’s drug and alcohol services and staff within Camden’s INTs.
- Build and maintain strong relationships with partners, including:
- Primary Care Networks
- Adult Social Care
- Mental Health Services
- Housing & Community Health providers
- Voluntary, Community & Social Enterprise (VCSE) organisations
- Support development of integrated referral pathways to enhance service accessibility and improve outcomes.
- Promote the value of drug and alcohol services within multidisciplinary teams.
- Foster joint working approaches, including:
- MDT (Multidisciplinary Team) participation
- Shared care planning
- Joint case visits
- Identify and resolve systematic barriers to seamless integration.
- Represent CGL’s Substance Misuse service at INTs, partnerships, and operational meetings.
- Collaborate with internal teams to embed integrated working models and bolster workforce development.

Candidate Requirements
We’re looking for candidates with:
- Significant experience in substance misuse services or supporting people with multiple disadvantages.
- Want-to-excellence in partnership working across health, social care, and community sectors.
- Deep understanding of referral pathways and integrated care frameworks.
- Strong relationship-building skills to engage diverse stakeholders smoothly.
- Track record of implementing service development or organisational change within complex systems.
- Clear, persuasive communication and seamless collaboration in multidisciplinary environments.
- The ability to diagnose systemic challenges and design practical solutions.

Salary
Base pay (pro-rata for part-time): CGL Points 36–40 (International Living Allowance/Over London Weighting/Fringe benefits apply)
- Salary range: £40,889.21 – £45,583.51
- Inner London Weighting: + £4,236.47
