About the Role
Join Leicester City Council (LCC) and our Outstanding-rated Integrated Crisis Response Service (ICRS). Recognised by the Care Quality Commission and central to the health and social care system, ICRS provides 24/7 emergency support for individuals in crisis within their own homes. As an ICRS Assistant, you’ll help pilot our new End-of-Life Care pathway and play a vital role in preventing hospital admissions by offering rapid, personalised crisis intervention.
The role involves being the first responder to crises, undertaking dynamic risk assessments, and assisting clients through stabilisation while always working within a 2-hour response window. As part of a close-knit team of 30+ members, you’ll collaborate with health professionals to ensure the best possible outcomes for vulnerable adults and older people with complex needs.
Key Features:
- Permanent, part-time role (30 hours per week).
- Mixed shifts (morning, afternoon, and evening) between 7am–10pm, covering 7-day rotas (weekends, bank holidays, and public holidays).
- Location: Neville Centre, Leicester General Hospital site.
- Acts as Trusted Assessor for equipment and assistive technology referrals.
- Pilot opportunity in our End-of-Life Care pathway.
This role does not offer sponsorship. Right to work in the UK must be verified prior to appointment.
Responsibilities
- First Response & Crisis Management:
- Support clients referred into the ICRS within 2 hours, including conducting dynamic risk assessments in real-time.
- Provide 24/7 back-up staffing where necessary.

Health & Wellbeing Checks:
- Measure oxygen levels, blood pressure, and temperature as needed.
- Assess people’s physical condition and offer fall-related assistance.

Personalised Support:
- Collaborate with clients to create personalised support plans following a stabilisation period of up to 72 hours.
- Work closely with Home First services (Reablement, Nursing, Therapy) to provide coordinated, strengths-based support.

End-of-Life Care Support:
- Assist and crisis-manage individuals on the end-of-life pathway, liaising with multidisciplinary teams to meet specific needs.

Safeguarding & Trusted Assessment:
- Identify and report all forms of abuse in line with safeguarding policies.
- Assess needs for key equipment and assistive technology to enhance independence at home.

Documentation & Communication:
- Maintain accurate progress reports, contact sheets, and medication records.
- Communicate clearly with clients, colleagues, and external teams using a supportive, empathetic tone.

Partnership Working:
- Engage with professional networks to align support (e.g., Social Care, Healthcare) and ensure proactive follow-up plans.
Requirements
Essential Skills & Qualifications:
- Experience in a health or social care environment, specifically with adults/older people with complex needs (e.g., physical disabilities, learning difficulties, mental health conditions).
- Strong understanding of safeguarding principles and ability to identify, record, and report abuse.
- Proven communication skills (both written and verbal) to facilitate strong relationships and effective partnerships.
- Excellent problem-solving abilities and proficiency in working under pressure and with tight deadlines.
- Commitment to learning new skills rapidly in a fast-paced, supportive environment.

Logistics & Additional Requirements:
- Full and valid UK driving licence; ability to travel between Leicester and Leicestershire.
- Enhanced Disclosure & Barring Service check (DBS).
- Reliability to work rota shifts, including nights, weekends, and holidays.
