The Role

The Post Holder Will

  • Be responsible for the development, management and delivery of an integrated rehabilitation and re-ablement community based team co-located within a locality.
  • Provide professional governance and leadership, within sphere of professional practice, to ensure compliance with professional and HCPC standards of proficiency.
  • As part of a multi-agency team, manage their own complex cases, using evidence based practice and person centred principles to assess, plan implement and evaluate interventions.

In This Key Role, You Will

  • Responsible for the line management of a multi-agency rehabilitation team in line with organisational values and behaviours.
  • Implements and monitors NLC & NHSL organisational and HR management policies and protocols.
  • Empower individuals to take responsibility for their own health and wellbeing by practicing in an open, transparent and inclusive manner; ensuring that people have the relevant information to participate in decisions about their care and treatment.
  • Support a culture of continuous quality improvement to ensure excellence in care and best value.

What You'll Bring

  • Degree in AHP profession/Social Work, supplemented by further training and short specialist courses to post graduate, level or equivalent.
  • Registered Member of the Health & Care Professions Council (HCPC)/Scottish Social Services Council (SSSC)
  • Previous experience working in a community or patient/individual setting.
  • Specialist knowledge, experience and continuous professional development of relevant conditions, pathology, policies and procedures associated with the area of practice.
  • A driving licence is required; this must be a full UK/EU/EEA licence.
