Registered Nurse – Intensive Support Team (IST)
We have an exciting opportunity for a registered nurse to join our Intensive Support Team (IST), part of the Wealden Neighbourhood Mental Health Team (NMHT), Specialist Older Adults Mental Health service based at Millwood, Uckfield, East Sussex.
You will play a key role within our multi-disciplinary service, supporting older adults with mental health problems, including dementia. This role involves working with individuals who have both complex mental and physical health care needs. As part of the team, you will:
- Support service users and their carers
- Collaborate effectively with internal Trust services and partner agencies
- Contribute to enhancing the lives of those under our care

- Autonomous case management, retaining continuing responsibility for a defined caseload of service users.
- Comprehensive assessments of individuals and their families, including those with high-risk presentations.
- Developing individualised care plans collaboratively with service users and carers.
- Providing specialist nursing assessments and advice as required.
- Managing ongoing risks in conjunction with the service user and carers, ensuring regular reviews and adjustments.
- Supervising junior staff and trainees where appropriate.
Person Specification
Essential requirements:
- Registered nurse with a qualifying nursing degree or diploma.
- Experience working with older adults and mental health needs, including dementia where applicable.
- Strong caring, compassionate, and communication skills.
- Ability to work collaboratively within a multi-disciplinary team.
- Commitment to evidence-based and creative practice to improve service user independence.
- Incumbent of, or eligibility for, NHS Band 6 grade.
