- Managing a caseload of social care assessments for adults and older people.
- Developing person-centred support plans aligned with needs and outcomes.
- Ensuring high-quality, legally compliant social work standards.
- Maximising independence, choice, and wellbeing.
Core Responsibilities:
- Complete and update annual reassessments and reviews.
- Identify and manage safeguarding concerns via assessments and referrals.
- Direct individuals to:
- Universal services.
- Voluntary sector resources.
- Local support networks.
- Update records and referral pathways, using systems like Mosaic.
- Be involved in targeted projects (e.g., Adult Early Help Team initiatives).
Note: Supervision is limited to student social workers and social care assistants as required.
Key Skills & Criteria:
Requirements:
- Must be a qualified social worker and registered with Social Work England.
- 2+ years post-qualification experience.
- Strong teamwork autonomy (able to function independently or collaboratively).
- Proven Mosaic, MS Teams, Outlook, Oracle proficiency.
- Physically capable of:
- Home visits (including stair accesses).
- Out-of-borough travel.
Security & Conditions:
- Satisfactory employment references and declaration of interest.
- Safer recruitment clearance (due to interactions with vulnerable adults).
- Enhanced Disclosure & Barring Service (DBS) check required.
- Continual registration with the regulatory body.
- Demonstrated fluent English for public-facing roles.
