Dorset Integrated Urgent Care Service (DIUCS) Advanced Clinical Practitioner

Dorset Integrated Urgent Care Service (DIUCS) is seeking an experienced advanced clinical practitioner to provide clinical leadership and expertise in our multi-professional team.

About the Role

As an established autonomous practitioner with highly developed communication skills, you will be making complex clinical decisions, planning and managing comprehensive assessments of patients across Dorset’s diverse patient population.

Responsibilities

  • Using your advanced clinical skills in developing and implementing the highest quality clinical assessment and treatment, you will be working across IUCS teams.
  • Assessing patients over the telephone / video consultation.
  • Managing patient referrals through the Single Point of Access Service (SPoA).
  • Visiting patients face to face with the out-of-hours service and urgent community response service (UCR).
  • Working in the IUCS Treatment Centres.

Requirements

  • Part-time position with a minimum of 23 hours per week.
  • Based at St Leonards HUB.
  • Role will cover evenings, weekends, and bank holidays as part of a 24/7 service.
  • 12-hour shifts are included within the shift pattern.
  • Independent Non-Medical/Supplementary prescriber.

  • Collect, collate, evaluate, and report information, maintaining accurate, confidential patient information.
  • Develop, implement, and audit protocols, guidelines, and policies and identify own personal developmental and educational needs.
  • Undertake clinical supervision and systematic peer caseload review.
  • Assessing patients utilizing telephone consultation and video consultation.
  • Assessing and treating patients in the community as part of the out-of-hours and Urgent Community response teams.
  • Working alongside the SPoA service, supporting the wider system with offering alternative pathways for patient care needs.
