Are you ready to help someone go home from hospital and live their life as independently as possible, while also being willing to communicate effectively with a variety of people? If so, this opportunity at Eastbourne District General Hospital could be for you.

The JCR In Reach service is a new initiative by JCR (a service supporting individuals regaining independence after accidents or illness) and has recently secured funding to become a permanent team. The role involves working within a small team at the hospital, participating in meetings to identify individuals ideal for discharge with JCR’s support.
About the Role
The JCR In Reach Co-ordinator will:
- Operate in a fast-paced environment, assisting people transition from hospital to their homes more efficiently.
- Communicate calmly and professionally with hospital staff, patients, and families (with consent).
- Liaise with family members/next of kin on behalf of patients to plan their home discharge.
- Contribute to goal-based support plans to help individuals regain independence.
- Collaborate closely with health/social care professionals and JCR’s multidisciplinary team.
- Build relationships with key stakeholders and work under the oversight of a Team Leader.
JCR’s work supports individuals across a short, yet intensive, timeframe. The ideal candidate can help accelerate recovery and enable patients to return to daily life with confidence.

You should have:
- Strong communication skills (written and verbal)
- A strong ethical framework and ability to empathise with people experiencing vulnerability
- Experience working with health and social care professionals (e.g., therapists)
- An ability to design, implement, monitor, and review individualised support plans
- A UK driving license with access to a vehicle—or—proof of willingness to meet travel arrangements for this role
Optional but considered positively:
- Relevant qualifications within care, health/social work, or predictive skills.
- Prior experience with staying-more-independent/rehabilitation programs.

How You’ll Work
The working pattern is generally 8:30 a.m. – 5:00 p.m., Monday to Friday. Occasional weekend shift participation may be required as part of a rota.

Salary & Employment Conditions
- Full-time pro-rata salary adjustments apply for part-time or term-time-only positions.
