Joint Community Rehabilitation (JCR) Support Worker – Job Description

About the Role

Are you ready for a fresh challenge and to help individuals like Sue regain their confidence and independence? Sue lost trust in her mobility after a hip injury, determined to walk her dog Daisy to the local park again.

The Joint Community Rehabilitation (JCR) service empowers people to achieve their most important life goals—whether that’s resuming daily routines, re-establishing mobility, or adapting to new circumstances. Our Support Workers assist older adults recovering from illness or injury by helping them regain independence in their own homes.

Working alongside multidisciplinary teams, your role may include:

  • Providing personal care assistance
  • Aiding with therapy exercises
  • Offering emotional support to rebuild confidence in managing everyday tasks

As a JCR Support Worker, you’ll provide transformative care to a diverse clientele. Your dedication will restore autonomy, uplift spirits, and enable meaningful contributions to their communities.


About Us

The Joint Community Rehabilitation service is at the heart of local care delivery, specialising in supporting older adults within their homes. Guided by collaboration and compassion, we integrate psychological, physiological, and practical care to help each person advance at their own pace. Our holistic approach respects individual needs while fostering a supportive culture for both clients and staff.


Responsibilities

Here’s a snapshot of the impact you’ll make:

  • Assistance with daily activities—e.g., mobility, dressing, meal prep—and therapeutic training to sustain progress
  • Collaboration with therapists, occupationational health, and healthcare networks to tailor recovery plans
  • Emotional and psychological support to combat isolation and rebuild self-esteem
  • Advocacy, ensuring client preferences and rights are prioritised
  • Networking within community resources for comprehensive support referral
  • Flexible travel across local areas as part of your duties

Skills, Qualities & Requirements

Ideal candidates will demonstrate:

  • Clear, collaborative communication with clients and care advocates
  • Patience and empathy—understanding life-limiting impacts and celebrating small victories
  • Teamwork, thriving in interdisciplinary settings
  • A valid UK driving licence and personal vehicle access to fulfil travel requirements

Opportunities & Career Growth

Contract Options

We offer flexible arrangements:

  • Permanent contracts – 6–30 hours per week
  • Apprenticeships – 24–30 hours (comprising work and training)
  • Weekend contracts – 6–24 hours
  • Evening-only roles – 6–24 hours, 7 pm–10 pm shifts

Salary

£26,824–£27,254 per annum (pro-rata for part-time or term-time). Hourly rate: £13.90–£14.13 depending on role and experience.

Note: No work visas or sponsorship are currently available for this role. Qualify for ESCC’s Refer a Friend initiative if applying via a discrection of a colleague.
