KM249448 - Community Mental Health Nurse

Band 6 Community Mental Health Nurse

An opportunity has arisen for a Band 6 Community Mental Health Nurse to join the Specialist Community Perinatal Mental Health Team within NHS Grampian.

This role involves providing enhanced, specialist mental health care to women experiencing moderate to severe and complex mental health difficulties during pregnancy and the postnatal period. You will be part of a supportive, experienced multidisciplinary team delivering high-quality, compassionate, person-centred care.

Main duties of the job

As a Band 6 CMHN within the CPMHT, you will:

  • Carry out specialist mental health assessments and contribute to formulation and care planning
  • Manage a defined caseload, providing enhanced community-based interventions and ongoing review
  • Assess and manage risk, working closely with the MDT and partner services
  • Develop and review individualized care plans, involving women and families wherever possible
  • Support infant mental health and the developing parent–infant relationship
  • Work collaboratively with colleagues across health, social care and third-sector services
  • Maintain clear, accurate clinical records in line with professional and organisational standards
  • Contribute to service development, audit, and quality improvement activity

You will work with a high level of professional autonomy, supported through regular supervision and clear clinical governance arrangements.
