KONIECZNE DOKUMENTY

Komplet dokumentów prosimy o dostarczyć w formie papierowej na dyżur.

Dyżury będą odbywać się w Sali EM27 w budynku B Wydziału Elektrotechniki i Automatyki Politechniki Gdańskiej (ul. Jana Sobieskiego 7). Harmonogram dyżurów znajduje się poniżej.

Niezbędne Dokumenty

  • Potwierdzenia przelewu za składki członkowskie (dla nowych członków)
  • Potwierdzenie przelewu za kurs
  • Wypełniony formularz zgłoszeniowy
  • Wypełnioną wg wzoru deklarację członkowską (dla nowych członków)
  • Podpisaną zgodę na przetwarzanie danych osobowych (ostatnia strona Karty Informacyjnej Kursu)
  • Podpisaną informację o zapoznaniu się z Kartą Informacyjną Kursu (ostatnia strona Karty Informacyjnej Kursu)
