Position Overview
We are seeking a highly motivated and experienced Lead Discharge Facilitator (permanent, full-time, 36 hours per week) to work within Ward 4, Woodland View Hospital.
Ward 4 is a specialist functional assessment ward for individuals aged 65+, providing care for older adults experiencing mental health difficulties.
Key Role:
- Deliver leadership in discharge planning to maximise bed use and ensure timely, safe discharges.
- Coordinate collaboration between inpatient services, community teams, and partner agencies.
- Reduce delays in admissions and unnecessary hospital stays.
Responsibilities
The Lead Discharge Facilitator will:
- Coordinate discharge planning from Ward 4, ensuring safe transfer of patients to homes or appropriate care settings.
- Collaborate with ward staff, community teams, families, and partner agencies to resolve complex discharge arrangements.
- Liaise with the Psychiatric Liaison Service, contributing to assessments and reviews of patients in acute settings awaiting Ward 4 admission.
- Provide short-term community support to patients awaiting admission to stabilise them in the least restrictive environment.
- Serve as an intermediary between acute hospitals, inpatient services, and community teams, maintaining a patient-centred, recovery-focused care approach.
- Champion person-centred, dignity-focused, and independence-oriented care for older adults with mental health challenges.
- Lead discharge planning strategies, guidelines, and institutional change initiatives to enhance safe and timely discharges.

Hours & Working Conditions
- 36 hours per week, predominantly Mon–Fri (0900–1700)
- Eligible for reduced working hours from 1 April 2026 (e.g., 36 hours instead of 37 for part-time teams without earnings reduction)
- Permanent position
Requirements & Qualifications
Required for this role:
- First-level Registered Mental Health Nurse with at least 3 years’ experience at Band 6.
- Master’s-level education or equivalent skills/experience.
- Mandatory experience: Both inpatient and community elderly mental health for older adults.
- Specialised understanding of older adult mental health (functional disorders, complexities in later-life care).
- Strong collaboration skills across boundaries to lead and resolve complex discharge cases.
- Demonstrated excellent communication, coordination, and leadership in risky or multi-agency settings.
- Commitment to person-centred, recovery-oriented care and reducing avoidable hospital admissions.
