Lead Mental Health Practitioner
Professional Lead – Mental Health & Wellbeing Team
Band 7/8a (Equivalent) | Directly Employed by the PCN
About the Role
Would you like to work within an innovative, forward-thinking Primary Care Network (PCN)? We are introducing a new hybrid role—merging the responsibilities of a Senior Mental Health Practitioner and a Lead for our Mental Health & Wellbeing team.
The role is directly employed by the PCN at a Band 7/8a level (depending on experience) and offers an exciting opportunity to help shape the future of primary care mental health.
Key Responsibilities
- Provide clinical leadership to the Mental Health & Wellbeing team, ensuring seamless integration across two practices.
- Work closely with the GP Mental Health Lead and wider multidisciplinary teams.
- Prescribe or develop prescribing capability once in post.
- Support pre-referral discussions with secondary care and strengthen cross-system collaboration, improving local mental health transformation.
- Build key partnerships with therapies (e.g., CBT, low-intensity interventions, MHBAT and neigbourhood teams).
- Ensure the PCN provides holistic, timely and evidence-based first-contact mental health support.
- Contribute to system-wide improvements by developing links with community, voluntary and neighbourhood services.

Work Scope & Commitment
- Part-time, full-time or job-shared applications considered, but at least 3 days per week recommended for effective engagement.
- Oversee development of the PCN’s social prescribing offer.
- Support duty teams with immediate mental health triage and management.
- Proactively develop the role—helping to establish a proposed Mental Health & Social Care Hub in North Kerrier.
Qualifications & Experience
- Qualified and registered Nurse (with the NMC).
- Clinical leadership experience, ideally in mental health or primary care.
- Prescribing qualification or willingness to work towards this.
- Experience in primary care or health systems transformation preferred.
- Commitment to autonomous collaborative working, time management and training.
