Lead Transition Officer
National Star College is seeking an experienced Lead Transition Officer to join our Transition Support Team.
This is an exciting opportunity to play a key role in supporting learners with complex needs to achieve positive and meaningful outcomes as they move on from college.
Role Overview
Reporting to the Transition Support Team Manager, you will work collaboratively with learners, families, colleagues and external partners to coordinate successful transitions into further learning, employment, volunteering, housing, social care provision and community opportunities. You will provide high-quality information, advice and guidance, ensuring learners are supported to achieve their aspirations and make informed decisions about their future.
Responsibilities
- Build and maintain strong relationships with long-term living, education, employment and community activity providers, helping to develop local and national networks that support learner progression.
- Contribute to transition reviews and planning processes, ensure outcomes align with Education, Health and Care Plans (EHCPs), develop accessible resources and information, and support the ongoing delivery of high-quality Information, Advice and Guidance services across the college.

Requirements
We are looking for someone with experience of providing information, advice and guidance and working with a wide range of stakeholders and agencies within an education / training environment, bringing good knowledge of benefits and support services available to disabled people. Ideally you will also demonstrate a background of working with young people with learning disabilities and/or additional needs.
- Excellent communicator with strong organisational and administrative skills.
- Ability to work independently, and confidence in building effective professional relationships.
- Proficiency in using IT systems to record and manage information are essential.
- Experience within multidisciplinary teams and an Information, Advice and Guidance Level 3 or above qualification would be advantageous.

Working Conditions
We can offer flexible hybrid working arrangements and would ask in return that you are available to meet the needs of the role including travel to our sites across Cheltenham, Hereford, Worcester and South Wales as required.

Contract: Full time 37hrs, all year 52 weeks
Salary: £28000 - £31000 DOE per/annum
Location Status: Hybrid 50:50
