Job Description: Learning Disability Liaison Nurse

To assist and support the Senior Learning Disability and Autism Clinical Nurse Specialist (CNS) in working collaboratively across the multi-disciplinary team to increase access, reduce health inequalities and improve health outcomes for people with learning disabilities and autistic people who use services at the Royal Orthopaedic Hospital (ROH).

Role Overview

The Learning Disability Liaison Nurse is responsible for delivering high-quality, patient-centred care to individuals with learning disability and/or autism diagnoses. The role focuses on assessing needs, planning and delivering care, and supporting patients to maintain independence, dignity, and wellbeing.

Responsibilities

• Undertake comprehensive assessments, develop and implement personalised care plans, and provide therapeutic and emotional face to face support to patients and their families.
• Work collaboratively within a multidisciplinary team to ensure coordinated care, promote recovery, and to safeguard vulnerable individuals.
• Work as part of the Vulnerabilities Team and support the Senior Learning Disability and Autism CNS.
• Be a highly competent and knowledgeable practitioner within the learning disability specialty and provide specialist learning disability and autism knowledge to support patients to reduce health inequalities.
• Actively promote the Learning Disability Nurse role to service users, families and carers and other professionals.
• Work with children, young people and adults who are diagnosed with learning disabilities, profound and multiple learning disabilities and/or autism, their parents, and carers to develop reasonable adjustment plans for outpatient services ensuring the provision of safe, high-quality services.
• Work as part of a MDT under the supervision of the Senior Learning Disability and Autism CNS to assist in the planning, implementation, and evaluation of comprehensive care plans pre-operatively, during admission and post-operatively ensuring a safe and effective discharge using a patient-centred approach.
• Be an advocate for patients within the Trust under the Learning Disability and Autism service.
• Escalate all complex cases to the Senior Learning Disability and Autism CNS for oversight.
• Provide face to face support and advice ensuring reasonable adjustments are in place to support the patient’s needs.
• Ensure the hospital passports are used and available across the Trust.
