Leeds: Care Coordinator

Care Coordinator

Location: Leeds Salary: £23,800.50 per annum Hours: Full-time (37.5 hours per week) Reports to: Care Manager

About the Role

To ensure all client visits are allocated, recruit suitable Care and Support Workers, and maintain appropriate office procedures.

Principal Responsibilities

  • Answer all incoming telephone enquiries
  • Implement thorough recruitment, vetting, and onboarding of Care & Support Workers
  • Process new client referrals
  • Prepare documentation for Field Care Supervisors
  • Permanently allocate Care & Support Workers to all client visits, ensuring 100% coverage
  • Administer Individual Care & Support Agreements
  • Maintain client files, Care & Support Worker files, and ensure full documentation is up-to-date
  • Update computer systems and electronic records at all times
  • Ensure compliance with all office systems and procedures
  • Assist with wages and invoice processing
  • Produce reports and management information as required

Key Qualifications & Expectations

Requirements

  • Experience in care or a related field (preferable)
  • Strong customer service skills
  • Passion for delivering person-centred care
  • Eligibility to work in the UK
  • Ownership of a valid UK driving licence and own vehicle (required for visits)
