We have an exciting opportunity for a Clinical Reconnect Practitioner to join our team in Humberside!
Hours - 37.5 hours per week
3x12hr shifts over 7 days - includes weekend and bank holidays
Salary - This position is full time and you will receive a salary of £43,609 to £49,175 per annum
Location - Humberside Region
Candidates are required to have a Legal right to work in the UK due to the nature of the role, offers of employment will be subject to additional vetting and security checks.
What you'll be doing
As a Liaison and Diversion Practitioner, your responsibilities will vary. You will:
- Be part of an extended team working across Liaison & Diversion service
- Work autonomously in court and police detention cells in order to identify and respond to requests of screening and assessment of people with identified health needs in contact with the criminal justice system
- Use validated tools to assess for a number of identified health needs and utilise information accordingly to inform decision-making and facilitate diversion within and out of the criminal justice pathway, creating a clear individualised care plan

What we'll look for in you

Qualifications, skills and experience required:
- Qualified Registered Mental Nurse or Mental Health Social Worker with current registration with the relevant professional body
- Substantial evidence of continuing professional development and post registration training.
- Experience of psychosocial interventions and awareness of substance misuse
- Excellent communication skills and ability to gain appropriate level of consent
- Understanding of clinical governance, health and safety, QOF, EBP and national standards
- Knowledge of evidence based practice, health promotion, NICE guidelines and pharmacology
- Knowledge of current legislation relating to community mental health provision and forensic psychiatry
