  • Contract: Permanent/Full-time – 40 hours per week
  • Salary: £52,000 per annum + £1,000 working from home allowance

The Role: Liaison/Triage Practitioner

You’ll join our established Liaison Nurse Team (LNT), supporting both general mental health triage and the Northeast and North Cumbria (NENC) Neurodevelopmental Triage Service. This varied, clinically fulfilling role demands expertise in mental health assessment, risk management, safeguarding, and patient-centred care.

Primary Duties Include:

Core Responsibilities

  • Provide specialist clinical support to Psychiatrists, Prescribers, and multidisciplinary teams.
  • Conduct mental state and risk assessments during telephone/remote consultations.
  • Perform timely triage and risk assessments, adhering to Psychiatry UK policies and national guidelines.
  • Triage referrals for neurodevelopmental assessments (ADHD, autism) using evidence-based criteria.
  • Make independent clinical decisions ensure safe, personalised care.
  • Develop risk management plans, escalating concerns as needed.
  • Maintain accurate electronic patient records and clinical documentation.
  • Promote shared decision-making and inform patients on treatment options.
  • Collaborate with NHS partners, external agencies, and internal teams for coordinated care.
  • Improve triage/neurodevelopmental services via process/quality-driven feedback.
  • Undertake safeguarding assessments and compliance with legal/operational standards.
  • Balance a priority-driven workload while preserving clinical excellence.

Responsibilities

As Our Liaison/Triage Practitioner, You Will:

  • Deliver expert telephonic assessments to ensure accurate referrals and safeguarding.
  • Use clinical judgement to triage referrals for neurodevelopmental (ND) panels or non-ND pathways.
  • Interface with NHS CCDs and commissioned care providers for continuum support.
  • Foster collaboration between Psychiatry UK, third-party clinicians, and families.
  • Apply latest guidelines (NICE, NHS DE, local commissioning source) rigourously.
  • Locally champion advanced digital mental health solutions and personnel CPD.

What We’re Looking For

Essential Criteria

You must demonstrate: ✔ Active registration with the NMC (Registered Nurse) or HCPC (Occupational Therapist). ✔ At least 3 years’ post-qualification mental health experience (or equivalent safe independent clinical decision-making skills). ✔ Evidence of professional development (CPD). ✔ Proven experience in mental health triage/liaison or neurodevelopmental care. ✔ Holistic risk assessment and safeguarding composition. ✔ Advanced IT for remote practice (video platforms, electronic documentation). ✔ Autonomy and organisational effectiveness. ✔ Book-a-Role DBS clearance ( supplemental voluntary DBS).

Desirable Experience

  • Postgraduate qualifications/mental health specialization.
  • Experience with ADHD/Autism assessments, primary care, or crisis triage services.
  • Familiarity with remote or digital triage models.
