Responsibilities
will include but are not limited to:
- Collaborate closely with RNs and physicians to ensure patient-care plans are followed; immediately report any changes in a patient’s condition.
- Educate patients and caregivers on self-care techniques to promote independence and well-being.
- Measure and document vital signs (e.g., blood pressure, temperature, pulse) during visits.
- Perform medication reconciliation–review all medications at each visit.
- Conduct specialized procedures as prescribed by physicians (e.g., foley catheter changes).
- Maintain aseptic technique when preparing equipment and materials.
- Complete comprehensive and accurate documentation in compliance with regulatory standards.
Requirements & Qualifications
- Successful completion of an LPN program and 1+ year of clinical experience.
- Active, unencumbered LPN licensure in the state(s) where the role is assigned.
- Current CPR certification (must be maintained while employed).
- Valid driver’s license, reliable transportation, and personal liability insurance.
- Strong commitment to quality patient care and adhering to clinical best practices.
