Job Title: Integrated Community Mental Health Service Lead

About the Role

This is an exciting opportunity to lead an innovative new integrated community mental health service. You will play an integral role in further developing the service, teaching and training new team members and primary care staff, and providing clinical and managerial leadership to the team.

This post has become available due to the previous postholder leaving. Please note the service is continually evolving, and the configuration of staff and exact job responsibilities may change to support development.

Key Responsibilities

  • Play a key role in a liaison/consultation model to provide early, easy access advice to GPs and Primary Care Networks (PCNs) to support the care of patients in the locality.

  • Provide clinical leadership in:

    • Clinical multidisciplinary team (MDT) meetings
    • Referrals meetings
    • Case discussions, including supervision for team members in and outside meetings
    • Assessments, particularly in risk and/or medication management
    • Diagnostic assessments
  • Establish, develop, and maintain positive working relationships with:

    • Colleagues in Primary Care Networks
    • Secondary care mental health teams
    • Improving Access to Psychological Therapies (IAPT) services
    • Local Authorities
    • Third sector organisations

  • Facilitate smooth transitions into and out of the service through collaborative relationships with other relevant services.

  • Lead the MINT (Mental Health Integrated Network Team) in partnership with the Team Manager by:

    • Guiding strategy development
    • Leading quality improvement initiatives
    • Conducting audits
    • Developing and refining written policies and guidelines for service delivery
  • Participate in training, including:

    • Teaching key individuals in the Primary Care Network, such as GPs
    • Formal, didactic teaching
    • Clinical teaching (e.g., “bedside” case discussions)
