Macmillan Palliative Care Clinical Nurse Specialist - Band 7 - 30 hours

The post holder will provide specialist palliative care advice and support for patients receiving palliative and end of life care who have complex symptoms associated to their life limiting condition. Responsibilities include referral and clinical caseload management. pain & symptom management advice and support, Assessing patients in both the Community and Hospital settings. Provide supervision and mentorship to team members band 6 and below. In addition, liaise with clinical colleagues in the Hospice and primary care where required, to provide support and coordinate care.

A high level of existing clinical skill and experience in managing palliative and end of life care patients to acting as a resource, support and educator to colleagues, clients, carers and other health and social care professionals.

  • Excellent communication skills will be essential in order to provide high quality information to the patient and you will need to be confident in undertaking significant and sensitive conversations and breaking bad news.
  • Ability to travel independently and efficiently across multiple community sites to meet the operational requirements of the role. The Service operates 7 days a week and the post holder will be expected to be included in the rota to cover weekend and Bank holidays.

Provide evidence-based specialist palliative care, advice and support to patients with a diagnosis of cancer or other life limiting conditions, and their carers. Working collaboratively with multi-professionals across primary and secondary and social care agencies including voluntary organisations

  • Lead service development and educational audit and research projects
  • Provide specialist palliative care advice, education and training to all professional groups of the multi professional team across all care settings as the service requires.
  • Provide line management to band 6 and below staff
  • Support the service in being compliant with CQC standards and ensuring their practice is evidence based and promotes patient safety.
