Mental Health and Wellbeing Practitioner

An exciting opportunity has arisen for a Band 6 Community Psychiatric Nurse to join our dedicated Waltham Forest Mental Health & Wellness Team Central, based at the Jane Atkinson Centre. This permanent role offers a salary within Band 6 (£39,959 - £48,117) and involves working 37.5 hours per week. With a start date of 7 October 2026, this position provides the chance to make a meaningful impact in the lives of individuals experiencing diverse mental health and wellbeing needs. The Role

As a Mental Health and Wellbeing Practitioner, you will play a vital role in supporting individuals with a range of mental health needs within a community setting. Working collaboratively as part of a multidisciplinary team, you will deliver timely, responsive, and recovery-focused interventions that empower service users to achieve their goals. This role also involves engaging with carers and relatives to ensure a holistic approach to care.

  • Undertake holistic assessments and collaboratively formulate individual care plans with service users.
  • Build and maintain effective relationships with service users, carers, and relatives, ensuring their voices are heard and respected.
  • Participate in case conferences, reviews, and multidisciplinary meetings to provide high-quality, evidence-based care.

Essential Skills And Qualifications

  • Demonstrated ability to assess service users’ needs and develop person-centred care plans.
  • Strong communication skills, with the ability to engage effectively with service users, carers, and professionals.
  • Knowledge and application of relevant policies, procedures, and legislation, including the Mental Health Act.

The post holder will play a key role in assessing needs, planning and delivering interventions, and supporting individuals to improve their mental health, resilience, and overall wellbeing. You will work collaboratively with service users, carers, and partner agencies to ensure a holistic approach to care, promoting recovery, independence and reducing reliance on secondary services.

The successful candidate will demonstrate excellent communication skills, sound clinical judgement, and a commitment to delivering safe, effective, and compassionate care.

This role is ideal for someone who is proactive, flexible, and passionate about improving access to mental health support within the community, while contributing to continuous service development and innovation.

Main duties of the job

You will be working as part of our multi-disciplinary team and will support the Team triage and coordinate complex and challenging cases coming into the service.

  • We require good IT skills and knowledge of using electronic recording systems.
  • It is essential that you are willing to work flexibly within the demands of the service.
  • You will work closely within a multi-disciplinary team and other agencies to deliver recovery focused health and social care interventions that includes managing risk and the Safeguarding of vulnerable adults.
  • The successful candidate will be expected to provide managerial supervision to junior colleagues, mentor MDT students and participate in ongoing educational and research programmes.

Health & Safety

Employees must be aware of the responsibilities placed upon them under the Health & Safety at Work Act (1974) and subsequently published regulations to ensure that the Trust’s Health and Safety policies and procedures are complied with to maintain a safe working environment for patients, visitors and employees.

Infection Control

Every employee has a duty to take personal responsibility for the prevention and control of infection, in accordance with Trust Infection Prevention and Control Policies, which reflect the statutory requirements of the Health Act 2006 Code of Practice for the Prevention and Control of Healthcare Associated Infection. All mandatory training must be attended and compliance maintained.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes. The induction will be held at our head office.

Higher cost Area Supplement

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,551 to a maximum of £5,735).

Data Protection

Personal data is protected under the Data Protection Act (1999) and the post holder will ensure that it is securely held and that the requirements of the Act are followed. It is the responsibility of all staff whose jobs requires them to record information in Trust Computer systems (e.g. RIO and ESR) to ensure that the data entered into these systems is of high data quality and that information is recorded correctly and in a timely manner. Failure to adhere to this requirement could be considered a disciplinary matter.

  • To undertake assessment of clients’ needs and formulate with the SU where possible individual care plans.
  • Build effective relationships with service users.
  • Ensure that relatives and carers of service users, where appropriate, have a good understanding of the condition of the service user and feel able to communicate effectively with the clinical area to answer any queries or concerns.
  • To participate in reviews and case conferences and attend ward/community reviews and CPA meeting.
  • To inspire hope in service users to achieve their recovery goals.
  • Spend constructive, designated time with service users on a 1-1 basis, to build and maintain effective relationships with them.
  • Communicate, at all times, in a way which demonstrates the Trust’s values and expectations when working alongside service users/carers and relatives.
  • Work alongside service users and carers/relatives to continually work towards improving and maintaining high standards.
  • To initiate and develop new ideas and methods in consultation with the Team manager.
  • Participate in research, audit reviews and other initiatives in accordance to Clinical Governance.
  • Actively contribute to the development and review of policies and procedures based upon best practice
  • To understand and practice all relevant Trust and local policies and procedures.
  • Raise quality issues and related risks with ward/team manager or senior manager.
  • Evaluate the quality of own work, making improvements when necessary.
  • Have a good understanding of the Trust’s Service User Led Standards, Quality and Performance agenda and how it impacts on the performance of their team.
  • Be responsible for ensuring that accurate legible records are kept in accordance with Trust policies and legal requirements.
  • Report all complaints and/or incidents in accordance with Trust procedures and ensure that the line manager is informed.
  • Cooperate with any investigation within the Trust.
  • To ensure that the application and administration of the Mental Health Act is implemented appropriately.
  • To actively identify full range of needs pertaining to equality and diversity issues that may arise for service users and their carers.
  • Ensure equality and diversity issues are reflected in the course of clinical practice and in carrying out all other duties pertaining to the post.
  • To be aware of any religious/cultural needs of service users and ensure this is reflected in the care that is provided.
  • To treat service users, relatives and carers with dignity and respect at all times.
  • To act in a way which facilitates service users rights to make their own decisions and also recognise their responsibilities.
  • To complete, with service user except in exceptional circumstances, a care plan, taking into account the service user’s needs and aspirations in order to promote recovery and well-being.
  • To establish supportive and empowering relationships with the Service Users and carers.
  • To take part in multidisciplinary and multi-agency assessments of service users and assist in coordinating regular clinical reviews.
  • Refer service users to other practitioners when needs and risks are beyond own scope of practice.
  • To maintain written progress notes and support programmes which are evaluated on a regular basis with the service users.
  • To ensure carers are offered assessment of their needs in a timely manner.
  • To ensure the safe custody of administration of drugs in accordance with professional code of practice and Trust policy.
  • To identify any problems or deficit of care provision, and take appropriate action.
  • Provide high quality evidence-based and individualised mental health care to a group of service users, relatives and carers.
  • Work alongside service user in identifying and achieving planned outcomes.
  • To undertake collaborative and holistic assessment of clients’ needs and formulate individual care plans and risk assessments to address identified needs and concerns.
