Job Overview
We are seeking a motivated and compassionate Mental Health and Wellbeing Practitioner (MHWP) to join our Older Adults Community Mental Health Team. This is an exciting opportunity to support older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty, within a multidisciplinary community setting.
The post holder will work collaboratively with service users, families, carers, and colleagues to coordinate person-centred care, promote recovery, and support positive wellbeing outcomes. Under the supervision of experienced mental health professionals and a Clinical Psychologist, the successful candidate will deliver evidence-based, psychologically informed interventions aligned to cognitive behavioural principles, including behavioural activation, problem-solving, emotional wellbeing support, sleep interventions, and relapse prevention planning.
This role offers the opportunity to develop specialist skills in older adult mental health while contributing to a compassionate, strengths-based service that values dignity, inclusion, collaborative care planning, and evidence-based practice.
Main duties of the job
The Mental Health and Wellbeing Practitioner will work within the Older Adults Community Mental Health Team, supporting older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty. The post holder will work collaboratively with service users, families, carers, and the multidisciplinary team to develop and deliver person-centred, strengths-based care plans that promote recovery, wellbeing, and independence.
Key responsibilities include:
- Undertaking risk assessments and contributing to risk management plans
- Gathering and analysing information to inform assessment, formulation, and care planning
- Acting as a named worker for allocated service users
- Coordinating care to promote continuity and positive outcomes
- Delivering evidence-based, psychologically informed interventions under the supervision of a Clinical Psychologist, including behavioural activation, graded exposure, problem-solving, emotional wellbeing support, sleep interventions, confidence building, medication support, and relapse prevention.

Detailed Job Description And Main Responsibilities
The Mental Health and Wellbeing Practitioner will work within the Older Adults Community Mental Health Team, supporting older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty. Working as part of a multidisciplinary team, the post holder will contribute to assessment, care planning, risk management, and the delivery of person-centred, recovery-focused care.
The post holder will work collaboratively with service users, families, and carers to develop strengths-based care plans, coordinate interventions, and support shared decision-making. They will provide evidence-based, psychologically informed interventions under the supervision of a Clinical Psychologist, including behavioural activation, problem-solving, emotional wellbeing support, sleep management, confidence building, medication support, and relapse prevention planning.
Key responsibilities include:
- Carrying out risk assessments
- Monitoring service users’ progress
- Maintaining accurate clinical records
- Contributing to multidisciplinary discussions
- Liaising effectively with healthcare professionals, social care, and third-sector organisations to ensure continuity of care

Qualifications
Essential criteria
- Bachelors degree at 2:2 in any subject for entry to Postgraduate route
- Postgraduate certificate (Level 7) for Mental Health and Wellbeing Practitioners (Adult Specialist Mental Health)
Knowledge/Experience
Essential criteria
- Experience of work interacting with the public
- Experience working as part of a team
- Experience of analysing and communicating complex information verbally and in writing
Desirable criteria
- Lived experience of mental health issues/difficulties
- Experience that supports working with service’s diverse local communities (from work, volunteering, or personal experience)
Communication/Relationship skills:
Essential criteria
- Able to establish and maintain empathic, supportive relationships with people in significant distress or who are cognitively impaired, their families, and carers
- Able to receive, understand, and communicate confidential client information of a sensitive and often complex nature, including discussing care with family members within boundaries of confidentiality.
- Able to communicate in a sensitive and reassuring manner, with empathy, and where appropriate reassurance.
- Able to make good use of clinical supervision in a group and/or individual format
- Liaise with other teams and services including external agencies as required for the wellbeing of service users.
Desirable criteria
- Ability to provide care to service users in languages spoken locally in addition to English
