Mental Health Liaison Practitioner
Band 6 | Full Time – 37.5 hours per week
Mid Sussex Neighbourhood Mental Health Team | Haywards Heath
£39,959 – £48,117 per annum (pro rata for part time)
£2,000 Golden Hello or Relocation Incentive available* Closing date: 20 July 2026
Work at the interface between primary and secondary care, supporting GPs and primary care teams to provide timely, person-centred mental health support while helping people access the right care at the right time.

We're looking for:
- Registered Mental Health Nurses (RMN)
- Social Workers
- Occupational Therapists
with:
- At least three years' post-qualification mental health experience
- Experience managing complex caseloads
- Strong assessment and risk management skills
- A collaborative approach to multidisciplinary working

The role
As a Mental Health Liaison Practitioner, you'll act as the vital link between primary care and specialist mental health services.
You'll:
- Assess people with complex mental health presentations within primary care
- Provide specialist advice and consultation to GPs and primary care teams
- Support risk assessment and care planning
- Ensure people are directed to the most appropriate mental health service first time
- Build strong relationships across primary care, secondary care and community services
- Deliver training, consultation and clinical support to primary care colleagues

Based within the Mid Sussex Neighbourhood Mental Health Team in Haywards Heath, working across GP practices and primary care settings throughout the local area.
