Occupational Therapist – Community Enhanced Rehabilitation Team (CERT)
About the Role
An exciting opportunity has arisen to join Mersey Care’s Community Enhanced Rehabilitation Team (CERT) as an Occupational Therapist.
CERT works with individuals in long-term rehabilitation placements, assisting their transition to the community or suitable alternative placement where required. The team also supports inpatient services, helping with capacity and flow. CERT provides enhanced care to individuals experiencing severe and enduring mental illness, ensuring their occupational therapy contributes to sustained recovery and community integration.
As part of a multi-disciplinary team, the occupational therapy role is integral to the team’s mission of delivering responsive, timely, and evidence-based interventions in a fast-paced, dynamic environment.
Key Responsibilities
Working Collaboratively Across Disciplines
- Work in a professional and collaborative manner with the Multidisciplinary Team (MDT)
- Participants in the MDT duty system, including undertaking duty assessments as necessary
- As the care-coordinator, fulfil the Enhanced Care Coordination (ECC) framework, including planning and coordinating:
- Client conferences
- Service reviews

Delivering Services in Diverse Settings
- Provide occupational therapy interventions in a range of settings to meet client needs
Supporting Carers and Professionals
- Offer information, education, and support to family members, carers, and relevant professionals
Monitoring Quality and Compliance
- Intral-agency collaboration to ensure positive working relationships across services
- Participate in case load supervision to:
- Monitor the standards of service-user care
- Ensure record keeping aligns with Professional and Trust standards
- Engage in regular clinical supervision with the Clinical Lead Occupational Therapist, adhering to Trust policy
Driving Service and Client Outcomes
- Contribute to the development and implementation of integrated care pathways, including:
- Assessments/referrals
- Models of care
- Collaborate with the whole team on:
- Cognitive behavioural therapy (CBT)
- Psychosocial interventions
- Compile and maintain effective client records, recording outcome measures in accordance with Professional and Trust policies
