Term: Fixed - 12 Secondment for existing LPFT staff, 12 month fixed term for external applicants
An excellent opportunity has arisen for a full time Band 5 CPN to join our Locality Mental Health Team supporting the North area of Lincoln.
About the Role
We are looking for a highly motivated RMN who is keen to join a supportive and recovery-focused service, working with service users experiencing severe and enduring mental ill health.
Responsibilities
- Work within a Multi-Disciplinary Team which includes Community Mental Health Nurses, Social Workers, Recovery Practitioners, Mental Health Wellbeing Practitioners, Psychological Intervention Facilitators, Nursing Associates, Community Support Workers, Psychiatrists, Psychologists and Admin staff.
- Collaborate with Neighbourhood Teams, Integrated Placement Based Teams, Adult Social Care, GP surgeries, and other partner organizations to ensure the best outcomes for patients and carers.
- Committed to a Recovery-focused way of working, inherent within the team culture, empowering and instilling hope to our service users.
- Provide high-quality person-centred care to patients, families, and carers.
- Act as Lead Healthcare Professional, undertaking comprehensive evidence-based assessments.
- Support service users to work towards recovery goals, build self-resilience, and promote independent living.

- Excellent communication skills are required.
