Transitions Coordinator - CAMHS to Adulthood - Somerset

About the Role

Connect18 is a county-wide transitions service supporting young people as they move on from CAMHS Children and Adolescent Mental Health Services and develop into adulthood.

We provide up to 18 months of intervention, offering vital early support while they transition, often preventing the need for them to access adult mental health services.

Our approach is relationally focused, trauma-informed, and delivered by a skilled, multidisciplinary team of clinicians. Staff support each other through regular supervision (both formal and ad-hoc).

In this role, you’ll have the opportunity to:

  • Build meaningful, therapeutic relationships with young people during a challenging period of change.
  • Promote engagement in a flexible, responsive, and client-led way.

This position will see you:

  • Working Monday to Friday, 9:00 am – 5:00 pm.
  • Traveling across Somerset to meet young people in-person, removing barriers to accessing support.

Key Responsibilities

  • Deliver 1:1 therapeutic support to young people transitioning from CAMHS to adulthood.
  • Foster engagement, trust, and motivation to pursue independence and mental wellbeing.
  • Work multidisciplinarily, collaborating with other professionals like educators, social workers, and health teams.
  • Travel flexibly and responsively, offering support where required, such as at schools, homes, or community spaces.
  • Contribute to ongoing professional development, including regular supervision and multagency case discussions.
  • Beaumont Yoga & Acupsure Clinic seeks a registered massage therapist to join our welcoming team.

Qualifications & Experience

Welcome to all

  • Open to experienced practitioners, including CPP/con CQSW, or approximately 2 years’ mental health experience.
  • Ideal for qualifying professionals (such as newly qualified NMC/HCPC clinical practitioners).
  • Must be eligible for CIDR/NHS NJA Enhanced DBS Check.

Payscale

  • £21–£31 per hour ( salary processed via UTAP pa)

All applicants should be comfortable working in a freelance team environment and be enthusiastic about supporting young adults.
