Mental Health Practitioner

Band 6 Registered Mental Health Nurse – Urgent Care Services

Post Details

  • Contract: Fixed-term, 3 months (to cover maternity leave)
  • Location: Salford Listening Lounge
  • Hours: 24-hour service (includes shift work)

About the Role

The Salford Listening Lounge is a 24-hour community space offering friendly, supportive care to individuals in Salford referred by healthcare professionals for urgent mental health assessments without requiring medical treatment.

Core Purpose: To deliver 1:1 holistic mental health assessments in a relaxed, non-clinical environment—reducing reliance on emergency departments. Key tasks include:

  • Offering brief psychosocial interventions to foster coping strategies
  • Collaborating on care plans for community management
  • Referring service users to appropriate local support

Though non-treatment-focused, this role demands clinical nuance, risk assessment skills, and excellent rapport-building.

Responsibilities

  • Referral Screening & Gatekeeping:

    • Carefully assess and filter urgent care referrals to ensure the least restrictive intervention options are prioritised.
  • Comprehensive Assessments & Care Coordination:

    • Complete detailed mental state and social needs assessments, collaborating with service users to develop individualised care plans.
    • Conduct comprehensive risk assessments, understanding drivers of behaviour, and implementing tailored formulations and interventions.
  • Keyworking & Service User Relationships:

    • Act as key worker for service users, offering consistent, empathetic support.
    • Collaborate with the multi-disciplinary team (MDT) to support shared care pathways.
  • Shift & Operational Coordination:

    • Occasional coverage of shift coordination/duty duties, ensuring seamless service delivery around the clock.
  • Liaison & Referrals:

    • Work closely with external agencies, ensuring smooth discharge/transfer of care to appropriate services.
