An Exciting Opportunity

An exciting opportunity has arisen to recruit a Mental Health Practitioner to our community mental health team in Withington, covering Wythenshawe. This is an excellent time to join our teams as we move towards a transformed clinical offer in our community services. You will be an integral part of shaping the new community provision along with our service users and carers.

Responsibilities

Postholders will offer timely and appropriate interventions to service users with enduring mental health needs in the Manchester area.

Mental Health Practitioners will be working in line with standard operational procedures demonstrating respect of individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.

  • To demonstrate high-level communication skills.
  • Be a full member of the multi-disciplinary team (MDT).
  • Effectively manage own caseload accepting referrals from agreed agencies in accordance with agreed operational procedures.
  • Ensure compliance with all Care Programme Approach requirements as per Trust Policy.
  • To participate in the continuous improvement of the service provided by the CMHT.
  • Manage a defined caseload of clients of any age within the Community Mental Health Team with complex physical and mental health needs in the community, using evidence-based/client-centred principles to assess, plan, implement and evaluate interventions. Record all interventions in line with relevant clinical pathways (functional or dementia) as outlined within the CMHT operational procedure.
