Role Overview
Surrey and Borders is looking for someone who is passionate about ensuring a high-quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community support to enable them to exercise choice over their own lives.
Location
Unither House, Curfew Bell Rd, Chertsey KT16 9FG
Working Pattern
2 days/week from 9 AM to 5 PM

Key Responsibilities
- To undertake detailed assessments of service users needs, developing individual
- To be responsible for sharing best clinical practice and to initiate innovative practice using evidence based research.
- To have the clinical knowledge and skills to provide individual care plans.
- To develop and maintain excellent links with all stakeholders.
- To establish therapeutic relationships and collaborative partnerships in order to ensure best working practice.
- To take responsibility for self-development and training updates with support from the Clinical Lead.
- To act as a role model for support workers and new team members.
- To contribute knowledge, skills and expertise to the overall performance of the team.
- To support fellow team members and maintain and develop the philosophy and values of the team.
- To keep well informed, to listen and respond to the views of colleagues, service users and their carers and encourage feedback to the team.
