A Local Authority mental health social work team is seeking an experienced Social Worker to work alongside health colleagues within a Community Mental Health Team based at Dewi Sant Health Park, Rhondda Cynon Taf. What will your responsibilities be? Assess and support citizens within a mental health setting Work collaboratively with multidisciplinary health teams in a CMHT Complete Social Services and Well-being (Wales) Act assessments and support planning Carry out Best Interest (BI) decisions and Mental Capacity Act (MCA) work Lead on safeguarding enquiries and interventions Provide duty cover as required Attend Quality Assurance panels for funding provisions Support effective hospital discharge processes and community transitions Qualifications and Experience: To be successful in this role you must have; Proven experience within mental health or CMHT settings Strong working knowledge of the Social Services and Well-being (Wales) Act, MCA, and safeguarding frameworks Ability to manage a complex caseload in a fast-paced environment Excellent assessment, communication and decision-making skills Available to start immediately Why Liquid Personnel?
