Our North & Mid Outreach team operate throughout the Burgess Hill/Haywards Heath/Crawley/East Grinstead areas so being a car driver is essential for this role.
Job Opportunity
As Part Of Our Outreach Team, You Will
- Provide personalised, community-based support to individuals with complex mental health needs, promoting independence within their own homes.
- Work closely with clients to implement and review tailored care plans, ensuring their safety and well-being.
- Collaborate with multidisciplinary teams, including clinical professionals, to deliver trauma-informed and recovery-focused care.
- Maintain accurate records and report any changes in client condition or concerns promptly.
- Participate in ongoing training to develop skills in mental health support and person-centred approaches.

Essential qualifications include strong communication and interpersonal skills, empathy, reliability, and a willingness to learn and work flexibly within a community setting. Previous experience in social care or mental health support is desirable but not essential.

Essential Skills
We are seeking Support Workers who are:
- Strong communication and interpersonal skills to effectively engage with individuals and multidisciplinary teams.
- Empathy and a genuine commitment to supporting people with complex mental health needs.
- Reliable and a professional attitude, including punctuality and accountability.
- Willing to learn and adapt within a flexible, community-based work environment.
- Able to work independently and as part of a team, maintaining a person-centred approach.
- Possess a basic understanding of mental health, trauma-informed care, and recovery principles is advantageous.
- Competent in maintaining accurate records and reporting changes in client condition promptly.
- Committed to ongoing professional development and training to enhance skills and knowledge.
Essential
- Car owner/driver is essential for this role.
Desirable
- Experience supporting individuals with mental health conditions in community settings.
