Salary: A GBP 25,000 per annum

About The Role

Are you passionate about supporting people to improve their mental wellbeing and overcome complex challenges? We are looking for a dedicated and compassionate Mental Wellbeing Practitioner to join our team at HMP Chelmsford.

In this rewarding role, you will work directly with adults experiencing co-existing mental health and substance misuse issues, providing tailored support through both one-to-one and group interventions. You will play a key role in helping individuals improve their wellbeing while working as part of a multi-disciplinary team within the prison environment.

Key Responsibilities

  • Manage a caseload of individuals with complex mental health and substance misuse needs.
  • Deliver individual and group-based mental wellbeing interventions.
  • Undertake assessments, develop care plans, provide advice and guidance, and make referrals to appropriate services where required.
  • Build positive and effective working relationships with partner agencies to ensure coordinated support for service users.
  • Attend multi-agency meetings, representing Forward Trust and providing updates on service delivery and outcomes.
  • Contribute to achieving organisational and commissioner targets, with a strong focus on measuring positive outcomes for those accessing the service.
  • Maintain accurate records and ensure all work is delivered in line with professional standards and organisational policies.

About You

You will be a compassionate, organised, and resilient practitioner with excellent communication and interpersonal skills. You will have experience of supporting individuals with mental health and/or substance misuse needs and be confident working collaboratively with a range of professionals to achieve the best possible outcomes.
