Mobilisation Lead
About the Role
We're seeking a passionate mobilisation lead to drive our transformational shift from residential care to supported living at Sense.
Working alongside the Head of Mobilisation, Director of Transformation, and operational leaders, you’ll turn strategic plans into action—collaborating directly with services, commissioners, and families to deliver person-centred change. This is a hands-on delivery role, where problem-solving, relationship-building, and safeguarding transformation are core.
Key Responsibilities
Lead Transformation Delivery
- Design and execute mobilisation projects accelerating the shift from residential care to supported living.
- Translate strategic decisions into actionable plans with clear timelines, ownership, and milestones.
- Work side-by-side with managers and frontline teams to embed change and refine operational workflows.
- Monitor progress, mitigate risks, and facilitate smooth transitions into long-term operations.
Keep People at the Heart of Change
- Ensure all transformations are person-centred, focusing on needs, aspirations, and rights of the individuals we support.
- Collaborate with families, advocates, co-production teams, and operational staff throughout the process.
- Deliver change with honesty, respect, and care, acknowledging the emotional and practical impact on people’s homes, routines, and relationships.
Build Strong Partnerships
- Coordinate cross-functional teams (operations, property, finance, HR, digital, communications) to achieve project goals.
- Develop trusting relationships with commissioners, housing providers, local authorities, and partner organisations.
- Lead workshops and planning sessions to drive momentum and practical outcomes.

Deliver Sustainable Services
- Support the creation of safe, financially viable supported living models.
- Contribute to business cases, transition plans, and service improvements, considering:
- Property & tenancy structures
- Staffing & funding models
- Regulatory compliance
- Referral processes & future demand
- Use data, feedback, and insights to inform decisions and enhance future transformations.
About You
We’re looking for an experienced transformation or mobilisation professional with a proven track record in delivering meaningful change. Ideal candidates will have:
- Expertise in leading or coordinating service transformation, mobilisation, or operational improvement in care/disability settings.
- Deep knowledge of supported living, residential care, health/social care policies, or disability services.
- Frontline management experience, including working with multidisciplinary teams to implement practical, grassroots change.
- Strong stakeholder management skills, particularly with commissioners, local authorities, and housing partners.
- Understanding of service sustainability, budgets, and commercial viability.
- Exceptional planning, analytical, and communication skills (both verbal and written).
- Confidence using digital project tools (e.g., JIRA, Power BI, SharePoint).
- A genuine commitment to person-centred practices, inclusion, and improving outcomes for disabled people.
