Stockport NHS Foundation Trust is delighted to offer an exciting opportunity for an enthusiastic and motivated Advanced Clinical Practitioner (ACP) to join our Neighbourhood Team within the Urgent Neighbourhood Services Directorate.
We are looking for an ACP who holds a Master’s degree in Advanced Clinical Practice (or equivalent) and is passionate about delivering innovative, compassionate care. If you thrive on challenge, embrace continuous learning and are committed to making a real difference in patients’ lives, we would love to hear from you.
This rewarding community-based role offers the chance to work autonomously as part of a multidisciplinary team, delivering high-quality, patient-centred care to people within their own homes and local communities. Working within your scope of practice and the Advanced Clinical Practice framework, you will provide advanced clinical assessment, diagnosis, treatment and care planning, using expert judgement, clinical decision-making and professional leadership to achieve the best outcomes for patients.
