Neighbourhood Nurse Lead

About the Role The Neighbourhood Nurse Lead is responsible for managing a community nursing team, including the district nursing team, in a neighbourhood. This role demands clinical leadership, expert advice, and supervision across a team while ensuring high-quality care for patients with complex health and social needs.


Key Responsibilities

  • Clinical Leadership & Supervision

    • Manage and provide clinical leadership to community nurses
    • Offer expert clinical advice and supervision to the team
    • Promote seamless patient care through collaboration with:
      • Service users
      • General Practitioners (GPs)
      • Social Care
      • The voluntary sector
  • Complex Caseload Management

    • Oversee management of patients with complex health and social needs on the district nursing caseload
    • Enhance patient experience across various treatment pathways

  • Preventive Care & Proactive Case Management

    • Work with GPs to assess, diagnose, and prescribe as needed
    • Deliver proactive case management to prevent unplanned hospital admissions
    • Champion patient safety and high-quality care
  • Health & Social Care Integration

    • Become the lead clinician for Health and Social Care integration
    • Advocate for patients and ensure their needs are met
  • Strategic Data Utilisation

    • Data stratification: Identify at-risk patients by analysing relevant data
    • Actively intervene where case management is required
