The Care Coach evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity.
Scheduled Hours
This is a remote position.
- The first 4 weeks of training will be from 8:30 AM to 5:00 PM EST.
- After training, the work hours are Monday-Friday from 10:00 am – 6:30 pm EST.
- No time off is permitted during the first month of training. The first 90 days following training are critical in building your foundation; therefore, planned or unplanned time away should be minimized as much as possible.
Key Responsibilities
- Conduct telephonic outreach (primarily via auto-dialer) to members with multiple chronic conditions and financial or functional barriers.
- Assess member needs through data review, structured assessments, and direct conversation to develop and manage individualized care plans.
- Coordinate services and connect members to appropriate clinical, community, and multidisciplinary resources to support optimal health outcomes. Ensure proper adherence to compliance.
- Identify and resolve barriers that impede effective care and member independence.
- Continuously monitor progress toward desired health outcomes and adjust care strategies as appropriate.
- Maintain strict adherence to compliance requirements within a highly regulated and highly visible program.
- Learn and navigate multiple computer systems simultaneously while engaging members in a fast-paced, structured environment with limited schedule flexibility.

Required Qualifications
- Associate degree in any course
- Minimum 2 years of experience in any direct adult patient care such as, but not limited to: long-term care, home and community-based services, hospital/facility setting, case management, managed care organizations
- Experience interacting with members of the healthcare team.
- Strong computer proficiency and proven ability to quickly learn and navigate a variety of electronic software programs, including electronic medical records.
- Exceptional communication and interpersonal skills to build rapport with internal and external customers and stakeholders.

Preferred Qualifications
- Bachelor's degree in an Allied Health Science.
- Experience working with elderly and/or chronically ill patients.
- Experience with Medicare and/or Medicaid patients.
- Experience conducting health assessments with patients.
- Experience assisting patients with health benefits and coordinating care.
- Ability to apply critical thinking skills.
- Experience with health promotion, coaching and wellness.
- Knowledge of community health and social service agencies and additional community resources.
Work at Home Requirements
To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Travel
While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$53,700 - $72,600 per year
