Head Nurse / Nurse Consultant – Frailty Assessment Unit (FAU) & Urgent & Emergency Care (UEC) Pathway Location: East Suffolk and North Essex NHS Foundation Trust – Colchester & Ipswich Contract Type: Full-time and part-time (7-day coverage needed)
About the Role
We are seeking an experienced, visionary, and transformative Nurse Consultant to lead and revolutionise care for older people within our Frailty Assessment Unit (FAU) and broader Urgent & Emergency Care (UEC) faecal system. This is a groundbreaking opportunity for an advanced practitioner proficient in clinical strategy, education, training, and service development—driving integrated care, avoiding admissions, and advancing outcomes.
Key Responsibilities
To act as an expert advanced clinical practitioner:
- Apply advanced clinical skills and knowledge
- Demonstrate critical thinking and research-based practice in managing your caseload
- Collaborate closely with consultants and specialist teams for professional guidance
To model and lead clinical leadership:
- Develop and champion a comprehensive Nurse-Led Model of Care
- Advocate for clinical competency across the trust and optimise patient care pathways
- Set and uphold high standards for nursing and advanced practice, embedding a robust governance structure for future advanced practitioners

Requirements
Qualifications & Experience:
- Advanced qualifications in nursing or advanced practice specialties relevant to frailty and urgent care
- Proven ability to bridge clinical practice, education, leadership, and service development in frail/elderly populations
- Evidence of system leadership, improved frailty pathway efficiency, and service transformation
- Experience in developing and implementing governance frameworks and staff education
Other Key Skills:
- Strong commitment to admission avoidance strategies
- Proven ability to embed digital health transformation initiatives (such as Epic EPR adoption)
- Experience in integrated care delivery models
- Competency in complex frailty assessments, including målläge (palliative care needs) principles
