Job Summary
Our Urgent Access Primary Care (UAPC) nurse delivers high-quality, patient-centred nursing assessment and care to adults presenting with urgent, non-emergency health needs within the UAPC service.
The post-holder will work autonomously and as part of the multidisciplinary team to assess, plan, implement and evaluate care, ensuring practice is aligned with professional standards, organisational policies and evidence-based guidance.
This new role supports the delivery of an integrated primary and urgent care model, enabling patients to access timely clinical advice, treatment and escalation or referral when required. By providing timely assessment, treatment and safety-netting, the post-holder contributes to effective clinical flow, continuity of care and positive patient outcomes across the wider system
Main duties of the job
We are recruiting for up to 67.5 UAPC nursing hours per week, with opportunities for full time, part time and casual (0 hours) currently available within the following service hours effective from 8th July 2026.
- Monday - Sunday 21:00 - 05:00
- Saturday - Sunday 09:00 - 17:00
Salary:
- £40,124.94 - £44,583.26 per annum FTE (37.5 hrs per week) -pro-rata for part time hours.
Location: Nelson House

Job Description
Job responsibilities
Main Responsibilities
- Deliver safe, effective nursing care to patients presenting with urgent care needs in community, clinic, or domiciliary settings.
- Undertake holistic patient assessments, including physical observations, clinical history, and risk assessments.
- Monitor, record, and escalate physiological observations in accordance with escalation protocols.
- Recognise clinical deterioration and follow escalation pathways
- Perform venepuncture and collect blood samples following procedures and infection prevention standards.
- Provide catheter care, including insertion (if trained), maintenance, troubleshooting, catheter changes, and patient education.
- Deliver basic wound care, including assessment, dressing selection, treatment planning, and ongoing evaluation of wound healing.
- Respond appropriately to patient falls, carrying out post fall assessments, neurological observations (where indicated), and escalation/referral as required. Use the lifting equipment after training
- Undertake verification of expected death in line with local policy, national guidance, and scope of practice, ensuring sensitive communication with families and carers.
- Undertake any other duties appropriate to the role and level of responsibility, as required to support the Urgent Access Primary Care service.

Skills
Essential
- Ability to undertake observations, venepuncture, wound care, and catheter care. Stoma care, basic wound care ECG.
- Ability to respond appropriately to falls and urgent clinical situations.
- Good communication, documentation, and organisational skills.
- Ability to work independently and as part of a multidisciplinary team

Qualifications

- Registered Nurse (Adult) with current NMC registration.
