Supporting Older Adults within the Trafford community

Supporting Older Adults within the Trafford community, who are experiencing a range of complex mental health issues. We work closely with families, carers and other health and social care professionals to promote recovery and autonomy.

We are a warm and friendly Multi-Disciplinary Mental Health Team with a strong focus on staff Health and wellbeing. To support staff wellbeing, we offer flexible working as per the trust policy.

About the Role

The Older Adults Community Mental Health Team in Trafford strives to provide quality recovery-based support to people with organic and functional mental health problems.

Under the line management and clinical supervision from within the CMHT, the post holder will coordinate OACMHT duty processes.

  • Coordinate the management of referrals
  • Provide timely assessments and interventions
  • Respond to duty calls from service users and other providers
  • Maintain the clinical management of referrals and assessments in line with the relevant clinical pathways within the CMHT operational procedure
  • Respect the individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs
