Occupational Therapist – Band 5 (Stroke Rehabilitation), Leeds Community Healthcare NHS Trust
About the Role
We seek a dynamic and passionate Band 5 occupational therapist specialising in stroke rehabilitation to join our supportive, multidisciplinary team. As part of the Community Stroke Rehabilitation Team, you will provide expert multi-disciplinary rehabilitation to stroke survivors in a community setting, working closely with patients’ homes and other appropriate environments.
The team collaborates with physiotherapists, speech and language therapists, psychologists, dietitians, nurses, therapy assistant practitioners, and rehabilitation assistants to deliver goal-focused, specialised interventions. Partners include Leeds Teaching Hospitals, third-sector organisations like the Stroke Association, and peers at every stage of the stroke pathway—from hospital to recovery at home.
This is an exciting time to join as we pioneer a new intensive rehabilitation pathway, enabling earlier hospital discharge while maintaining our existing high-quality service.
Key Responsibilities
- Design and implement a new intensive rehabilitation pathway, supporting timely patient discharge from hospital while maximising recovery.
- Deliver community-based stroke rehabilitation in patients’ homes and relevant settings.
- Work collaboratively within a multidisciplinary team (MDT), contributing clinical expertise and advice.
- Support unregistered team members, offering guidance and supervision to ensure high-quality care.
- Emphasise evidence-based practice to drive positive rehabilitation outcomes.
- Engage with external partners, including hospitals and charity organisations, to bridge care gaps.
- Weekend working may be required as applicable.
- Engage in service development and innovation, actively contributing to team improvements.

Person Specification Criteria
- Essential:
- Current registration with the Health and Care Professions Council (HCPC) as an occupational therapist.
- Demonstrated experience working with stroke survivors or neurological conditions (preferable).
- Proven ability in multidisciplinary team (MDT) collaboration.
- Community-based rehabilitation experience in best practice application.
- Strong clinic-skills with a commitment to evidence-based rehabilitation.
- Highly motivated individual, driven by improving patients’ quality of life.
- Desirable:
- Expertise in stroke-specific rehabilitation techniques.
- Leadership in team development or clinical supervision.
