Occupational Therapist
Occupational Therapist (Band 6) – East Cornwall (North Cornwall, Bodmin)

The Service & Team
We are seeking a dedicated, enthusiastic, and motivated Band 6 Occupational Therapist to join our Locality Therapy Team based in East Cornwall (North Cornwall, Bodmin).
Our team includes:
- Occupational Therapists
- Occupational Therapy Assistants
- Rehabilitation Officers for the Hearing Impaired
- Vision Rehabilitation Specialists
- Technical Officers
You’ll be working within a Primary Care Network (PCN)—embracing multi-disciplinary, locality-based, and strength-focused care models to support our health colleagues.
Key Responsibilities
You will assess, evaluate, and develop person-centred, strength-based plans to enable individuals with long-term physical or social care needs to maintain functionality, independence, and safety within their communities.
Core Duties:
- Conduct person-centred assessments to determine functional and occupational abilities.
- Adapt solutions to prevent, reduce, and delay care needs, promoting timely and appropriate support.
- Collaborate on environmental and functional home assessments, linking with reablement and rehabilitation services as needed.
- Provide information, guidance, and recommendations around:
- Care equipment provision
- Minor and major adaptations
- Rehousing advice to optimise independence and well-being.
- Generate cost-efficient, innovative, and practical solutions for holistic care.
- Conduct seating and moving/handling assessments where relevant.
- Advocate for optimal care following evidence-based practice for positive outcomes.
- Operate under statutory safeguarding principles.

Requirements & Qualifications
- Full professional qualification in Occupational Therapy (Diploma, Degree, Masters, or equivalent).
- Registration with the Health and Care Professions Council (HCPC).
- Minimum Band 6 experience as an autonomous Occupational Therapist, preferably within the NHS or Adult Social Care.
Critical traits include:
- Exceptional interpersonal skills and the ability to work effectively under pressure.
- Commitment to lifelong learning, sharing best practices across disciplines.
