Occupational Therapist – Band 5 (Intermediate Care Service)
About the Role
We’re seeking a motivated and enthusiastic Occupational Therapist (OT) to join our Intermediate Care service, covering community and hospital pathways. This role provides an opportunity for newly qualified or experienced Band 5 Occupational Therapists to develop clinically within a supportive and collaborative environment.
You’ll manage your own caseload, conducting assessments, delivering interventions, and working with health and social care teams to help people regain independence and return home safely. The team prioritises autonomy, ongoing learning, and service improvements, offering structured supervision, peer support, and career development opportunities tailored to your experience.
Ideal candidates are passionate about promoting independence, thrive in a fast-paced setting, and enjoy contributing to a proactive, values-driven team.
Key Responsibilities
In this role, you will:
- Manage a diverse clinical caseload to support patients regain independence and facilitate safe discharge from hospital or community pathways.
- Conduct holistic assessments and identify patient needs and risks.
- Plan and deliver person-centred interventions to maximise recovery.
- Collaborate with healthcare professionals (and caregivers) to optimise discharge planning and service flow.
- Prioritise tasks efficiently while adapting to fluctuating demand.
- Maintain accurate and clear clinical records.
- Delegate appropriately to therapy assistants, supervising their development and performance.
