Discharge to Assess (D2A) Occupational Therapist – York
If you’re an Occupational Therapist who thrives on fast-paced, person-centred work and wants to make a tangible difference to people living in York, this role could be the perfect next step.
The Opportunity
You’ll leverage your clinical assessment skills and a strength-based approach to support adults with diverse physical conditions. You’ll ensure safe, timely support for individuals discharged from hospital to a short-stay unit by:
- Conducting holistic assessments in both the short-stay unit and clients’ homes
- Developing and implementing tailored interventions
- Monitoring progress to optimise recovery and independence
Autonomy is encouraged, with guidance available from a supervising Occupational Therapist, while routine OT-specific CPD events will support your professional growth.

What You’ll Do
- Collaborate with social workers and the Multi-Disciplinary Team (MDT) to design person-centred support plans
- Perform functional assessments, including moving and handling assessments, across physical and medical conditions
- This position is job-share friendly
- Public-facing role: Must demonstrate C2-level English proficiency (conversational clarity and fluency in face-to-face advice-giving)

Contractual details:
- Your primary location will be the designated City of York Council office for your team
- No visa sponsorship available
Candidate Requirements
- Hold the necessary professional safeguarding checks
- Effective communication skills (mandatory spoken English CEFR C2)
- British English spelling retained
